Northern Neighbours Nurse Practitioner-Led Clinic

Working collaboratively and reporting to the Lead Nurse Practitioner (NP) and the Clinic Administrator, the Social Worker will deliver services to the clients of the Northern Neighbours NPLC by providing general counselling and other supportive functions for individuals, couples and families attending the clinic. The Social Worker functions collaboratively within an interdisciplinary team and strives to enhance the capacity of the clients to address their own needs. The social worker will support expertise and knowledge of the health promotion, disease prevention, and chronic disease management across a client’s lifespan. This may include social navigation of the social programs, advocacy and referrals as well as short -term case management.

The NNNPLC Social worker will be an integral part of a team of allied professionals. They will be encouraged to assist in establishing a health care clinic in a small northern community. The social work role holds potential for growth and has opportunities for career development through mentorship and education. The ideal candidate will enjoy living close to nature and be a self-motivated individual.

Duties and Responsibilities
• Represent the Northern Neighbours Nurse Practitioner-Led Clinic (NNNPLC) in a manner which promotes the mission and values of the organization.
• Counsel and make appropriate referrals for individuals, couples, and families attending the clinic.
• Conduct screening and assessment in areas of professional expertise related to health promotion, disease prevention, and chronic disease management across the client’s lifespan.
• Assist and where necessary, support client access to services in the above listed areas of focus.
• Work collaboratively with the individual, community, other professionals, and agencies to advocate for social change.
• Make recommendations regarding services and referrals in conjunction with the client’s interest, the social worker’s judgment and knowledge, appropriate legislation, and the NNNPLC policies.
• Inform clients of foreseeable risks as well as rights, opportunities, and obligations.
• Assist and refers families in the completion of necessary forms (e.g. Ontario Works, ODSP, housing and childcare).
• Seeks out resources and programs
• Practice in accordance with legislation relevant to social work practice.
• Establish and maintains clear and appropriate boundaries in professional relationships.
• Gather data for fiscal, statistical and analysis purposes.
• Maintains the strictest confidentiality.
• Participates in professional and personal development of self.
• Uses an electronic record to improve patient outcomes

• Registered with the Ontario College of Social Workers and Social Service Workers
• Satisfactory Criminal Records Search
• Ability to work flexible hours and travel as determined by the Lead NP or Clinic Administrator in relation to service requirements.
• Must possess a Class “G” Drivers License and have access to a personal vehicle.
