About Us:
The Northern Neighbours Nurse Practitioner-Led Clinic (NNNPLC) is seeking a compassionate and dedicated Registered Nurse (RN) to join our team in beautiful White River, Ontario, serving the communities of White River and Netmizaaggamig Nishnaabeg (formerly Pic Mobert First Nation). NNNPLC is committed to providing comprehensive primary healthcare with a focus on a holistic approach and collaboration.
The Role:
The RN will play a vital role in our multidisciplinary team, delivering primary healthcare services across the lifespan. You will work collaboratively with the Lead Nurse Practitioner to provide:
Chronic Disease Management: Assist patients with managing chronic conditions like diabetes, hypertension, and asthma.
Preventative Care Programs: Implement and participate in programs focusing on prevention and health education.
Health Promotion Services: Educate patients on healthy lifestyle choices and disease prevention strategies.
Health Assessment: Perform comprehensive health assessments and screenings.
Qualifications:
Registration and active membership with the College of Nurses of Ontario (CNO)
Minimum 2 years of experience in primary healthcare or a related setting
Excellent communication, interpersonal, and teamwork skills
Strong commitment to patient-centered care and cultural sensitivity
Proficiency in electronic medical records (EMR) software a plus
