Position Description
The Kenora Chiefs Advisory’s Primary Care Initiative is an exciting new development in the provision of health care for First Nations Communities. Kenora Chiefs Advisory is seeking a Float Nurse to provide holistic nursing services in KCA member communities.
Position Title: Float Nurse
Department: Primary Care
Salary: To be determined
Term: Full-time
Reports to: Nursing Manager
Summary of Duties
Ø Work closely with partner health care providers and consult with clients’ Primary Care Physicians, visiting physicians and Nurse practitioners on an ongoing basis
Ø Complete scheduling for visiting Physicians and Nurse Practitioners based on assessment data
Ø Manage patient clinics and flow
Ø Provide telephone and in-person triage
Ø Obtain immunization certification with ISC and Provide Immunizations
Ø Perform contact tracing and provide reports as per ISC for communicable diseases
Ø Provide homecare and perform comprehensive and focused health assessments that include health history, health status, risk factors and the social determinants of health and develop client care plans
Ø Conduct needs assessments for advanced care and medical directives, complete and document thorough medication assessments, review laboratory and diagnostic reports and flag potential concerns for visiting Physicians and Nurse Practitioners
Ø Provide interventions as per medical directives or medical orders, such as wound care, staple/suture removal, injections, ear syringing, assisting with procedures and specimen collection in accordance with the Ontario College of Nurses Standards of Practice
Ø Perform program evaluation and reporting, including collection and compilation of data in accordance with funding requirements, entering information into Community Based reporting templates, e-SDRT and E-HRTT, preparation of reports for community leadership and preparation of proposals when needed
Ø Engage with individual KCA member community leadership and health representatives to conduct needs/assets assessments
Ø Negotiate and assist individual First Nations communities with advocacy for equitable services and agreements with federal, provincial and regional service delivery partners in order to enhance Community Health Nurse and Primary Care Nurse programming
Qualifications
Ø Certification as a Registered Nurse or Registered Practical Nurse in the province of Ontario with good standing and membership to RNAO (or equivalent) with LAP
Ø Current CPR/AED with HCP and First Aid certification
Ø Experience in Public Health Nursing, particularly with communicable disease control and outbreak management
Ø Minimum of 5 years of nursing experience in a Primary Care setting
Ø Understanding of and ability to apply Clinical Practice Guidelines as well as other acts and legislation that govern practice
Ø Experience in program planning and evaluation and performance management
Ø Experience and understanding of Indigenous cultural issues, the geographical realities and social conditions within northern remote First Nations communities
Ø Knowledge of the Social Determinants of Health specific to First Nations Communities
Ø Ability to apply a bi-cultural lens to work activities
Ø Demonstrated understanding of PHIPA and ability to maintain client confidentiality
Ø Excellent organization and computer skills
Ø Strong written and verbal communication skills and effective organizational, critical thinking and problem-solving skills
Ø Knowledge of the Anishinaabe culture and language is considered an asset
Ø Ability to work both independently and as a team member
Ø Applicants should possess a valid driver’s license/insurance, have a vehicle and be willing to travel when required
