Job Summary:
The Diabetes Coordinator will take on a leadership role in diabetes education within Biigtigong Nishnaabeg, overseeing the daily operations of the Diabetes Program. This includes planning, implementing, and evaluating the program’s nutritional care component. The primary aim of this program is to enhance the quality of life for individuals affected by diabetes through education, support, and resources that help them manage their condition effectively.
Preferable skills & Qualifications:
A successful candidate must possess the following:
• Ontario Secondary School Graduation Diploma (Grade 12).
• Bachelor’s degree in food/nutrition is preferred, or a Community Health Diploma or equivalent
• Experience with community program planning and community needs assessment
• Clear criminal record check
• Valid driver’s license and reliable access to vehicle
• Certification in First Aid & CPR
Summary of Duties & Responsibilities:
One-on-One Counseling
• Develop, implement, and evaluate individualized nutritional care plans for community members living with diabetes.
• Educate clients on diabetes risk factors.
• Create educational sessions and tools focused on the signs and symptoms of diabetes.
• Provide resources and information on the different types of diabetes and their management.
• Teach personal care techniques for managing diabetes.
• Provide guidance on nutrition and its role in diabetes management.
• Develop meal plans that meet national standards for healthy meals.
• Educate on the importance of physical activity in diabetes management.
• Address the impact of diabetes on families and offer support.
Plan, Coordinate, and Facilitate Diabetes Prevention Programs
• Working in collaboration with Community Health Nurse to organize events such as the Poker Walk & Health Fair, Walk Your Mocs, and Diabetes School Programming in November.
• Coordinate diabetes screening activities throughout the year.
• Lead the planning and facilitation of a Diabetes Retreat and Diabetes Walk & Health Fair.
• Play a key role in implementing the Community Diabetes Prevention Strategy.
Coordinate and Facilitate Monthly Diabetes Support Group Meetings:
• Assess the needs of the support group and provide ongoing support.
• Arrange guest speakers and educational sessions for the group.
Implementation of the Biigtigong Nishnaabeg Diabetes Care Model:
• Update and revise the Diabetes Care Model as needed.
• Develop policies and procedures for the model with input from the health care team.
• Promote the Diabetes Care Model to other service providers and community members.
Serve as the Front-Line Contact for the Diabetes Program:
• Respond to client inquiries, provide information about services, and refer clients to appropriate service providers when necessary.
Plan, Coordinate, and Facilitate Healthy Lifestyle Activities/Events for the Community:
• Collaborate with coworkers to create Employee Wellness programs for staff.
• Coordinate and facilitate summer programs like Get Fit Club.
• Oversee the Biigtigong Mno-zhi Yaawgamig Walking Group, including activities like Urban Poling and Snowshoeing.
• Coordinate the Mno-miijim Mshkimod (Healthy Food Bag) program.
• Plan and facilitate school programming related to health and wellness.
Participate in Committees Promoting Community Wellness:
• Be an active member of the Diabetes Care Committee (Aanji Naanigedaa), working on health policy changes with organizations like Health Canada and FNIH.
• Contribute to the Health Promotion and Diabetes Committees in Marathon.
• Participate in the Accreditation process and be familiar with the role of the Diabetes Coordinator in quality improvement and accreditation efforts.
Collaborate with Program Partners:
• Coordinate placements for first-year medical students as part of the Integrated Community Experience.
• Establish and maintain strong working relationships with the Community Health Nurse, Community Health Representative, and other community resources to assist in coordinating health education activities.
Perform other duties as required
