Transitional Housing Support Worker


Position Summary

The Transitional Housing Support Worker provides day-to-day support and supervision to residents living in transitional housing. Working within a housing-first and client-centered approach, the worker helps maintain a safe, respectful, and supportive living environment while assisting residents with daily living activities, housing upkeep, medication support, and community integration. The role focuses on resident well-being, building positive relationships, and ensuring program expectations are met rather than providing formal case management services.


Key Responsibilities:

Resident Support

  • Build positive and professional relationships with residents.
  • Provide daily support and encouragement to residents in maintaining stable housing.
  • Conduct regular wellness checks and engage residents in meaningful daily activities.
  • Offer emotional support, active listening, and crisis de-escalation when required.
  • Encourage resident participation in life skills, recreational, and community-based activities.
  • Promote a respectful and inclusive living environment.

Medication Support

  • Assist residents with medication reminders according to program policies.
  • Monitor and document medication compliance where required.
  • Report medication concerns, missed doses, or health-related issues to appropriate staff.
  • Maintain accurate medication records and documentation.

Housing and Property Maintenance

  • Support residents in maintaining clean and safe living spaces.
  • Conduct regular room and unit inspections.
  • Assist residents with housekeeping tasks, laundry routines, and sanitation practices.
  • Monitor common areas to ensure cleanliness and safety standards are maintained.
  • Report maintenance concerns and safety hazards promptly.

Daily Operations

  • Complete shift reports, incident reports, and daily documentation accurately and on time.
  • Monitor building security and resident safety throughout shifts.
  • Respond appropriately to emergencies, crises, and critical incidents.
  • Assist with resident intakes, orientations, and move-outs.
  • Enforce program rules, expectations, and occupancy agreements in a fair and consistent manner.
  • Participate in staff meetings, training sessions, and program activities.

Community and Resident Engagement

  • Support residents in developing healthy routines and independent living skills.
  • Encourage positive community involvement and neighbour relations.
  • Assist residents in accessing transportation, appointments, shopping, and other daily living needs when appropriate.
  • Foster an environment that promotes dignity, respect, and personal responsibility.

Qualifications

  • Diploma or degree in Social Services, Community Services, Mental Health, Addictions, Human Services, or a related field preferred.
  • Experience working in housing, shelter services, mental health, addictions, or community support settings.
  • Knowledge of harm reduction, trauma-informed care, and housing-first principles.
  • Strong communication, conflict resolution, and interpersonal skills.
  • Ability to remain calm and professional in crisis situations.
  • Experience working with vulnerable populations.
  • Basic computer skills and ability to complete electronic documentation.
  • Current First Aid and CPR certification are considered an asset.
  • Must have mandatory COVID shots.


Working Conditions

Shift work includes:

  • Days 07:30 am to 03:30 pm
  • Evenings 03:30 pm to 11:30 pm
  • Nights 11:30 pm to 07:30 am
  • Weekends and holidays
  • Exposure to individuals experiencing homelessness, mental health challenges, addictions, and complex life circumstances.
  • Ability to respond to emergencies and physically patrol housing units and common areas.
