Job Summary:

Reporting to the Manager of the Mental Health and Addictions Program, Diabetes and Telemedicine, the Social Worker is responsible for coordinating, developing, implementing, and evaluating case management services within the RAAM Clinic. The role includes conducting staged screening and assessments, providing case
management and system navigation support for clients, facilitating group education sessions, and developing individualized treatment plans to support patients throughout all stages of their substance use recovery journey. The Social Worker also serves as an advocate for addictions services and harm reduction initiatives while promoting compassionate, client-centered care. Qualifications:
• Post-secondary education in Social Service Work, Social Work, or equivalent required.
• Registration with the Ontario College of Social Workers and Social Service Workers, or eligibility to obtain registration required.
• Experience supporting individuals with substance use concerns, including the ability to engage clients with empathy, compassion, and a harm reduction approach.
• Knowledge and understanding of local community resources, challenges related to substance use, and First Nations cultures and communities is considered an asset.
• Ability to assess client needs, analyze situations, and provide appropriate options, solutions, and resolutions.
• Strong interpersonal, communication, conflict resolution, and relationship-building skills, with the ability to work effectively within interdisciplinary teams and with patients, families, communities, and partner organizations.
• Ability to adapt and respond effectively in a dynamic and changing healthcare environment.
• Proficiency in Microsoft Office applications.
• Experience working within a healthcare organization is considered an asset.
• Training or experience in SMART Recovery, solution-focused brief intervention, relapse prevention, treatment planning, crisis or suicide intervention, and/or motivational interviewing is considered an asset.

Job Duties:
• Collaborate with the multidisciplinary RAAM team, patients, support persons, and community partners to identify client needs and develop individualized treatment plans.
• Provide trauma-informed, solution-focused brief intervention, case management, crisis intervention, system navigation, and referral services to RAAM clients in a compassionate and nonjudgmental manner.
• Deliver therapeutic support and facilitate rapid access to substance use and mental health services.
• Create, coordinate, organize, and facilitate substance use-related group programming, community education, promotion, and advocacy initiatives.
• Maintain current knowledge of community resources and services and work collaboratively within the client’s circle of care.
• Participate in program development, quality improvement initiatives, and ongoing evaluation activities.
• Maintain accurate patient documentation, statistics, and reports in accordance with hospital policies, professional standards, and ethical guidelines established by the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
• Advocate for individuals accessing substance use services and promote harm reduction, patient-centered care, and evidence-based addictions treatment practices.
• Act in accordance with the policies, procedures, and values of Sioux Lookout Meno Ya Win Health Centre.

Salary:  $69,225 – $83,343 starting. Commensurate with skills, experience and education.
