Position Overview:
The CA Family Wellness Worker traveling role is dedicated to supporting the health and well-being of families across diverse communities in Canada through direct outreach and culturally sensitive interventions. This position involves traveling to various locations to engage with families, assess their needs, and provide guidance and resources that promote holistic wellness. The role requires building trusting relationships with individuals and families, collaborating with community partners, and advocating for access to health and social services. The ultimate goal is to empower families to achieve improved physical, emotional, and social health outcomes by addressing barriers and fostering resilience. This position plays a critical role in enhancing community wellness by delivering tailored support in a respectful and inclusive manner across different regions.
Qualifications and Requirements:
- Post-secondary education in social work, community health, Indigenous studies, or a related field.
- Experience working with families and communities in a health or social services setting.
- Valid driver’s license and willingness to travel extensively within Canada, including remote and rural areas.
- Strong understanding of cultural safety principles and trauma-informed care.
- Ability to maintain confidentiality and adhere to ethical standards in client interactions.
- Experience working with Indigenous communities and knowledge of Indigenous cultures and traditions.
- Certification in First Aid and CPR.
- Additional training in mental health, substance use, or family violence intervention.
- Proficiency in multiple languages spoken within Canadian communities, such as French or Indigenous languages.
- Familiarity with electronic case management systems and data reporting tools.
- Travel to multiple communities within Canada to provide in-person family wellness support and outreach services.
- Conduct comprehensive assessments of family needs, including physical, emotional, social, and cultural factors impacting wellness.
- Develop and implement individualized wellness plans in collaboration with families and relevant community agencies.
- Facilitate access to health care, social services, and community resources by providing referrals and advocacy.
- Maintain accurate and confidential records of client interactions, progress, and outcomes in compliance with organizational policies.
- Build and maintain strong relationships with community stakeholders, including health professionals, social workers, and Indigenous organizations.
- Deliver culturally appropriate education and workshops on topics such as nutrition, mental health, parenting, and substance use prevention.
- Participate in ongoing professional development and team meetings to enhance service delivery and share best practices.
Hours of Work: 32.5
Salary Range:Â $33.48 To $42.41 Hourly
