Job Title: Nurse Practitioner
Location: Remote First Nations Communities in Ontario The NP reports to the Health Director.

Salary: $125,156 – $141,540 annually, plus retention bonus starting year 2 ($155,156 –

$171,540)

Recruitment bonus of $20,000 (paid in two installments at 6 and 12 months)

Position Summary
Shibogama First Nations Council seeks skilled and adaptable Nurse Practitioners (NPs) to provide culturally responsive, community-centered healthcare services to remote and isolated First Nations communities. This role combines onsite and virtual care to deliver holistic primary care, support health promotion, and prevent illness.

Full-time positions may include a virtual care component, where the Nurse Practitioner leads primary care virtual clinic days and provides consultation to Community Health Nurses (CHNs)

for CTAS 4 and 5. Remote work leverages secure telemedicine platforms, such as the Ontario Telemedicine Network (OTN), to deliver patient care and consultations from outside the community. NPs collaborate closely with in-community Community Health Nurses to ensure seamless patient care, including pre-visit preparations and post-visit interventions. Virtual care schedules and workflows are designed to maintain flexibility and efficiency, supporting both the NP and the community’s needs.

Part-time positions offer flexible rotations, including options such as two weeks on and two weeks off or month-long rotations. Opportunities for overtime are available during both in- community assignments and remote work periods.

The NP will collaborate with local healthcare providers, community leaders, and interdisciplinary teams to improve health outcomes and address systemic disparities in alignment with Shibogama’s mission.

Primary Responsibilities
·         Primary Care Delivery: Conduct comprehensive health assessments, diagnose illnesses, and develop care plans. Educate clients and families on disease prevention and healthy living.

·         Community Engagement: Build trust with community members and leaders to address unique healthcare needs and act as an advocate for holistic community health.

·         Collaboration: Work with physicians, nurses, and healthcare teams to deliver coordinated care, including referrals and shared decision-making.

·         Virtual Care & On-Call Duties: Provide remote consultation and guidance to Community Health Nurses for CTAS 4 and 5 and lead primary care virtual clinic days to enhance community access to care.

·         Leadership & Mentorship: Serve as a role model for clinical excellence, mentor new practitioners, and participate in orientation and training initiatives.

·         Program Development: Assist in designing and implementing health promotion programs tailored to community needs.

·         Documentation & Compliance: Maintain accurate health records, adhere to regulatory requirements, and support quality improvement efforts.

·         Displays excellent communication and organizational skills in responding to co-workers, as well as other community stakeholders and partners.

·         Promotes a positive image and reputation of the nursing program on behalf of the Tribal Council.

·         Follows set policies and procedures of the nursing program and of Shibogama First Nations Council.

·         Advocate and stand for the Tribal Council and Member Communities.

·         Support projects aimed at improving health systems through healthcare innovation, collaborative approaches, and interdisciplinary teams.

·         Exercise professional judgment and initiative while undertaking all responsibilities and developing relationships.

Education & Certification
·         Graduation from a Master’s degree program in Nursing or a recognized Nurse Practitioner

Program with a focus on Primary Care (All Ages) or relevant specialization.

·         Current registration as a Nurse Practitioner (Extended Class) in good standing with the College of Nurses of Ontario.

Essential Qualifications & Experience
·         Recent and significant experience providing Community Health or Primary Care nursing services in one or more of the following areas:

–  Community Health (Public Health and Home and Community Care)

–  Primary Care

–  Urgent or Emergency Care

–  Acute or Intensive Care

*Recent is defined as within the last five years and significant as at least two years of experience performing complex activities related to this task

·         Good working knowledge of computer applications including Microsoft Office and email systems.

·         Excellent organizational skills, professionalism, and time management.

·         Excellent verbal and written communication skills.

·         Strong interpersonal skills with co-workers, communities, and partners.

·         Ability to work independently and take initiative.

·         Ability to mentor others.

Asset Qualifications
·         Recent and significant experience providing Community Health or Primary Care services in:

–  Chronic Disease Management (including Mental Health or Addictions)

–  Pediatrics

·         Experience in working with First Nations communities

·         Experience as program or project leader in delivery of health services

·         Ability to communicate effectively in one of the dialects of the Treaty 5 and/or 9 Territories

·         Understanding of the Social Determinants of Health in Remote and Isolated Communities

·         Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment)

·         Valid Driver’s License

Conditions of Employment
·         Willingness and ability to travel in small aircraft, off road vehicles and by boat in varied weather conditions to communities in remote and isolated areas

·         Flexibility to work in a rigorous and often demanding work environment

·         May be required to be temporarily reassigned to other communities as operationally required

·         Willingness and ability to perform on-call duties, work after hours, and on weekends as required

·         Willingness and ability to complete mandatory education/training relevant to the position

·         Compliance with medical and security clearances, including immunizations
