About us:

We are a progressive Family Health Team located in the heart of the Canadian Shield halfway between Thunder Bay and Sault Ste. Marie on the North Shore of Lake Superior. Our team includes seven physicians, a nurse practitioner, a physician assistant, a midwife, nurses, a social worker, a health promoter and registered kinesiologist, an epidemiologist, administrative staff, and organizational leadership who work together to provide comprehensive, person-centred care.

We serve a local population of 3300 as well as residents from Biigtigong Nishnaabeg and Netmizaaggamig Nishnaabeg for a total population of approximately 4000.

Position Overview: 

The Social Service Worker (SSW) supports individuals, families, and groups in addressing a range of personal, interpersonal, and social challenges. Through assessment, intervention, and ongoing evaluation, the SSW applies social service work knowledge, skills, and strategies to promote well-being, build on strengths, and support clients in achieving their goals. This role involves working collaboratively within an inter-professional team and connecting individuals and communities to appropriate resources and supports to enhance overall functioning.

Duties: 

  • Assist patients with completing forms and accessing education, resources, and supports
  • Support system navigation and care coordination
  • Provide supportive care to individuals experiencing life transitions and challenges (e.g., grief and loss, changes in health status, financial or housing concerns, mental health or substance use challenges)
  • Collaborate with healthcare and community partners to provide consultation, share information, and support patient care
  • Contribute a social service work perspective to MFHT programs and services

Compensation Details:

In accordance with Ontario Health’s current wage and benefit remuneration for Social Service Workers, we offer the following wage and benefits package:

  • Annual Salary between a minimum of $54,394.95 and a maximum of $61,222.00.
  • HOOPP (Healthcare of Ontario Pension Plan)
  • Employer paid extended health benefits such as prescription, vision, dental, Long-term disability and AD&D insurance.
  • Continuing Professional Development annual benefit
  • Generous vacation and occasional personal illness paid benefit plans
  • Relocation expense budget of $2,500.00 (supporting documentation required)
  • $1,000.00 signing bonus (upon successful completion of probationary term)

Requirements:

  • 2-year College Diploma in social service work.
  • Member in good standing with the Ontario College of Social Workers and Social Service Workers.
  • Demonstrated ability to communicate effectively in English, both verbal and written.  French language ability, verbal and written also a definite asset.
  • Knowledge of relevant laws, standards and guidelines.
  • Excellent interpersonal, conflict resolution, communication and organizational skills.
