Positions Available: 1-Full-Time Contract Position (18 months)

Position Location: Thunder Bay, ON (this position is not remote)

Position Overview
Under the direction of the Diabetes/Chronic Disease Health Service Manager, the Certified Diabetes Educator will be responsible for the delivery of diabetes public health and education to First Nations communities. This will include the delivery of monthly education sessions, the coordination of quarterly screening clinics, researching current trends and prevalence of diabetes, reviewing, identifying, and addressing service gaps with respect to diabetes intervention, development and/or purchase of educational materials, and facilitating access to secondary and tertiary care where necessary. The Certified Diabetes Educator will positively impact the lives of the community members of the First Nation, provide community members with the education needed to manage their health, and provide current and research-based education to community members and serve the community by promoting positive health care changes.

Salary Range: $43.63 To $47.68 Hourly

Qualifications and Requirements

  • A BScN Degree is required
  • Good standings with the College of Nurses of Ontario
  • Diabetes Educator Certification is required
  • Diabetes/Advanced foot care certification is an asset.
  • Three (3) years’ experience in community-based service delivery, knowledge and experience in the delivery of public health and education programs, previous experience in the delivery of diabetes education.
  • Must be willing and able to work flexible hours, provide on-call service on a rotational basis
  • Must be certified in CPR and Standard First Aid
  • The regulated health care provider must provide evidence of registration with the appropriate regulatory body
  • Must possess be able to travel regularly, have a valid Ontario Driver’s License and have access to a personal vehicle
  • Criminal records search and vulnerable sector search

Hours of Work: 32.5 hours per week
