Position Overview:
Under the direction of the Counselling & Clinical Services (CCS) Manager, the Clinical Services Care Coordinator plays a key role within the Clinical Services team, ensuring children and youth receiving psychiatry services receive high-quality, client-centered care. This position involves working collaboratively within a multi-disciplinary team to address the mental health needs of children and adolescents, with a focus on complex and high-needs clients.
The Clinical Services Care Coordinator provides clinical guidance on mental health concerns, supports service coordination, and facilitates clear and effective communication with hospital physicians, specialists, and other community mental health providers. This role is essential in ensuring that clients receive seamless, efficient, and comprehensive care tailored to their unique needs.
Qualifications and Requirements:
- An Honors Bachelor of Social work is required.
- A minimum of three (3) years’ experience as a Social Worker with patients and families in a mental health setting.
- Demonstrated competence in team building, conflict management, and interpersonal effectiveness.
- Ability to navigate complex mental health systems and coordinate care across multiple providers
- Significant experience in pediatrics, children and adolescent mental health and/or addictions.
- Satisfactory Criminal Records Search; including a Vulnerable Sector Search.
- Ability to travel and work flexible hours as determined by the Service Manager or designate in relation to service delivery requirements.
- Must have a valid Class “G” Driver’s License and access to a personal vehicle.
Hours of Work: 32.5 hours per week
