Job Summary:
The Social Worker – Specialist Services plays a key role within a multidisciplinary team supporting the health and well-being of clients of Specialist Services. The focus of this position is to provide integrated, client-centered support through case coordination, clinical collaboration, and system navigation.
Working collaboratively with Specialists Services staff, the Social Worker – Specialist Services ensures high-quality care by contributing to assessments, providing follow-up, and strengthening links between families, clinical services, and community supports.
This position provides an opportunity to make a meaningful impact in a dynamic clinical setting, while working closely with skilled and supportive professionals dedicated to the wellness of children, youth and adults.
The Social Worker – Specialist Services reports to the Manager – Specialist Services.
Qualifications:
• Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment)
• Bachelor’s degree in social work (or a related field); master’s degree considered an asset.
• Registered and in good standing with the Ontario College of Social Workers and Social Service Workers.
• Minimum 2 years of experience in children’s and/or youth mental health.
• Experience in interdisciplinary environments, ideally including psychiatry or pediatrics.
• Knowledge of trauma-informed care and culturally responsive practices.
• Familiarity with the needs of Indigenous children, youth, and families and experience working in northern or remote communities.
• The ability to perform the requirements of the position on a regular basis.
Roles and Responsibilities:
1. Participates in treatment planning, care coordination, discharge planning, and clinical follow-up.
2. Works holistically to meet the physical, social, and emotional needs of clients.
3. Sensitivity to the needs of parents/caregivers and the ability to support and involve them in treatment plans.
4. Supports the work of specialists, together with the nursing and admin team, through
collaboration and case management. Case management functions include engagement, support, assessment and planning, education, community linking and care coordination, and advocating for options and services to meet client needs through communication and available resources. 5. Maintains accurate and timely clinical documentation in electronic medical record systems.
6. Considers all community/health resources that can be integrated into care. Forms and maintains strong connections to a multitude of First Nation community resources.
7. Demonstrates strong time management, reliability, and organizational accountability.
8. Exercises care and caution in protecting confidential and sensitive information related to clients and personnel
9. Participates in quality improvement activities and initiatives.
10. Upholds client rights and advocates for culturally safe, inclusive care.
11. Promotes teamwork and collaborative practices within the Specialist Services team.
12. Performs other duties as assigned by the manager.
